Getting Started with Weight loss Surgery 

  1. Contact us to schedule your first appointment.
  2. We will help you determine if you qualify for surgery. You will find general qualification criteria here. Our office staff is available to answer any questions you may have.
  3. We will obtain your insurance information to determine if weight loss surgery is covered under your specific insurance plan.
  4. Complete our Registration Packet and return at your appointment.
  5. If you prefer, you may call us to obtain more information and reserve a seat at a free educational seminar to be presented by Dr. Felts, prior to your first office appointment.  

Please bring the following items with you to your appointment:

  • Print out the Registration Packet forms below and fill them out. Bring them with you, or fax them prior to your appointment.
  • Bring your insurance card to each appointment.
  • Bring all prescription and over-the-counter medication that you are currently taking.
  • A list of all questions and/or concerns that you may have.
  • Please do not bring children under the age of nine to your appointment. This will ensure your full attention with the doctor.
