Depending on where you are on your weight loss journey, we offer two types of consultations. Our free 15-minute consults with Dr. Felts can help educate you on the various weight loss procedures and introduce the benefits of each. If you are ready for the next step in the process, Dr. Felts also offers a full personal consult based on your current and past health history and what procedure may be best for your needs. The fee for the full consult goes towards your surgery cost if you choose weight loss surgery with Dr. Felts.
Getting started on the right path is simple:
- Contact us to schedule your first appointment.
- We will help you determine if you qualify for surgery. You will find general qualification criteria here and our office staff is available to answer any questions you may have.
- We will obtain your insurance information to determine if weight loss surgery is covered under your specific insurance plan.
- Complete our Registration Packet and return at your appointment.
Please bring the following items with you to your appointment:
- Registration Packet materials (bring them with you or fax them prior to your appointment).
- Your insurance card.
- All prescription and over-the-counter medication that you are currently taking.
- A list of all questions and/or concerns that you may have.
- Please do not bring children under the age of nine to your appointment. This will ensure your full attention with the doctor.
